INDEPENDENT FACILITATION

Independent Facilitation Services

Independent facilitation services are available on an ongoing basis to help you and your circle of support to implement your person-centered plan and operate successfully in the Self-Determination Program. Your FMS will pay your independent facilitator out of your individual budget.

Independent facilitation services look different for everyone because it all depends on your goals and what you want your independent facilitator to do for you. The role of your independent facilitator might look different throughout the year based on what is going on in your life at that time.

Your independent facilitator can assist you with:

​

  • Locating and obtaining services and supports to meet your needs and goals, and/or developing creative options specifically for you.

  • Coordinating and managing your services and supports.

  • Connecting you with community resources and opportunities.

  • Making informed decisions regarding your individual budget.

  • Tracking your spending throughout the year and making changes to your spending plan as needed.

  • Leading, participating, or advocating on your behalf in the person-centered planning process and development of your annual IPP.*

  • Achieving individual goals: whatever you want to do with your life, your independent facilitator is there to help you make it happen.

 


*Note: your first person-centered plan is paid for by the regional center, but all future person-centered plans or updates to your person-centered plan are paid for out of your individual budget.
